Film Requests

To request films, please contact us weekdays from 9:00AM-4:00PM, allowing a minimum of a 48-hour advance notice for all procedures conducted at any Booth Radiology location.

(48-hour notice does not include weekends.)

The following information is required to process your request:

Patient name
Patient date of birth
Contact phone number
Patient procedure date (if possible)

Please call 856-848-4998, or toll free 877-BOOTH-99 (877-266-8499) to request films. For procedures performed at the Woodbury office ext: (1214), or for procedures performed at the the Washington Township office ext: (2415).

Please note: We do not currently accept film requests by e-mail.
